Quality Matters: Grant Application

Note: You cannot save this form, so please make sure you have all necessary information, including the name and OSU e-mail address of your Dean/Chair or Supervisor and Financial Officer, before you begin.

Upon submission of this grant application, an email notice stating that you are registering for the course and the responsibility for payment in the case that you do not complete the course will be sent to your department chair or dean and to your financial officer. You may want to speak with your department Dean / Chair before you submit your application to familiarize them with QM and your plans to take the Quality Matters workshops.

Note: The Office of Distance Education and eLearning will cover the registration fee only for those who successfully complete the QM workshops / courses. If you do not complete the course or if you fail to cancel your registration 24 hours before the course begins, you and/or your department will be responsible for payment of the registration fee.

The Office of Distance Education and eLearning Grants program is supported financially from the Wide Open West (WOW!) Affinity Program.
